I have used A-Ads.com as a publisher on a low traffic site that didn't make the miminums for most networks. I'm happy enough that I am rolling it out across other sites.

Signing Up: It is anonymous, and you just need a bitcoin wallet to sign up, you can even opt out of giving an email address.

Payment: There are two payment options: straight to the wallet with a minimum payout, or to Faucetsystem.com if you want no minimum. I use FaucetSystem and it has made regular daily payouts. What did impress me was that on a day when the ad unit was largely down, they still paid out the 5 satoshis it earned.

I attach payment proof from FaucetSystem.com for a couple of days, including the 16th August when it made 5 satoshis and still paid, showing it really does have no minimum.

Subdomains: I've used it on a subdomain and a comicsfury site. As long as you can access the html you can embed the ad. Just make sure that you use the full subdomain as the domain you register it against e.g. if you only want the ad in one wordpress category, you'll need to register it for that categories url or the ad won't activate. Once an ad unit is registered and active, you can use it on any site though.

Ads shown: The ads I get are mainly referral, but when you set the ad unit up you can choose what type of ads to show including NSFW, Shady etc. I blocked all questionable content, which is why my earnings are low. It also shows what your expected BTC earnings are as you change the settings when you configure the ad, which is useful. I haven't had any viruses or malware issues from them.

Problems: The Adaptive ad size doesn't work well, and the ads they serve didn't fit in the ad-unit. I had some cut off halfway along. It works much better if you set a fixed size for the ad-unit. Also, once an ad-unit is set up for a URL, that URL can't be changed. You can use the ad-unit anywhere but if that URL goes down the box will be deactivated everywhere.

Also in Anonymous mode, immediately after generating an ad, it will show an access code to update or edit your ad box below the embed code. Scroll down on a small monitor as it can be below the fold. Copy these details somewhere immediately, print it out, and keep it safe because if you lose this code you can't update the adbox. You will still be able to earn from it as withdrawals are automated, but not to change any settings or update it.

I have no idea.. I believe their rate varies all the time and depends on the number of ad campaigns and the active publishers.
I get around 500 visitors daily and I've earned 0.0001 btc for the past week

I think the rate is really great $0.50 per CPM, I've mostly used facebook or other costly paid ads though.

But I don't think a ads will allow traffic exchange as it must lose money, their rate is not low as ptp site.
