Banners, links it really doesn't matter, it matters how related is that page to the visitor browsing your site.
If it's a review about that program you promote there's a higher chance of conversion, hope it helps.

468x60 banner is better than 125x125 banner. The pricing is the same. Most of the ad networks where you can advertise your referral link accepts 468x60 banner in their homepage of their websites.
 
I don't think the banner outlook matters,the most important thing is to place the banners in copicious manner that it can easily be seen and accessible for visitors to click.Let banner ads be in consonance with the niche or content of the blog.

Size doesn't matter in this case, just placement. Most people have "banner blindness" anyway so i think if you split test the two and see which one gets more clicks - roll with that one.

Although sizes of banners don't affect a lot for most of cases, I personally prefer bigger than smaller. For me 728x90 is my best fave but things should be considered how fit it looks to the page you work on.
 
When you advertise with others, the bigger the better usually.
Exceptions apply of course.
A good placed link after an article can be much sweeter and much more profitable because it doesn't look like an advertisement anymore. A lot of people are "blind" to banners these days.
 
I think considering the adblockers are blocking the banners. not sure how the strategy is going to work out. but 300x350 seems to be a good option for those who are into the banners and the ads. I have seen some of the people working out with it.
